Ignition Interlock Provider Center Recertification
IGNITION INTERLOCK PROVIDER CENTER RECERTIFICATION
Ignition Interlock Provider Center certification is issued for a period of up to four (4) years. The Provider Center owner(s) should apply for recertification within 60-90 days of expiration.
IGNITION INTERLOCK PROVIDER CENTER OWNER RECERTIFICATION REQUIREMENTS
- Completed Program Recertification Application.
- All applicants, including owners, partners, corporate officers and/or controlling stockholders, must complete GAPS fingerprinting for a national and state criminal background check.
- All applicants, including owners, partners, corporate officers and/or controlling stockholders, must submit a certified Motor Vehicle Report (MVR) from each state in which they were licensed in the past 5 years except Georgia.
- Continuation or Verification certificate for the Surety Bond currently on file.
- Georgia general liability insurance policy not less than $50,000.
- $100 non-refundable application fee.
- Current agreement/contract between the manufacturer and the Provider Center owner signed by both parties within one year of applying.
- All customer contracts, invoices, and forms used by the Provider Center.

Applicant Processing System.

APPLICATION PROCESS
Please complete the application packet in its entirety to enable us to complete the process in a timely manner. It is advisable when mailing to use certified or mailing that can be tracked. Our goal is to completely approve your application within 30 business days. Once the application and supporting documentation are received, we will contact you within 7-10 business days.
