This is an online service that allows you to prepare the required license application form before visiting a Customer Service Center.

2. Is there a fee to use the License/ID/Permit Form?
There is no fee to use this online service.

3. Why should I complete this form?
The form is required when visiting a DDS Customer Service Center. It helps prevent fraud and prepares you for your visit.

4. How long will my information remain on file?
Your information will remain on file for 60 days. After 60 days you will need to complete and submit your information again.

5. What happens if I can't make it into the center before my information expires?
You will need to complete and submit the information again if the you cannot make it to a center within 60 days.

6. How important is my e-mail address?
We will use your e-mail address to send your confirmation and reminders.

7. Is it necessary for me to print?
You may print for your personal records, but only applicants under the age of 18 are required to print. However, we recommend you print the Real ID requirements page to ensure you have all the documents needed for your visit.

8. Do I have to be a certain age to submit the License/ID/Permit Form?
If you are under the age of 18, you are required to have a parent/ guardian, responsible adult or authorized driver training instructor to sign the form for your driver's license. You should bring an unsigned copy of your printed form to the DDS Customer Service Center.

9. How can I cancel what I submitted?
Once submitted, you cannot cancel. We will keep your record for 60 days.

10. What devices can I use to fill out the License/ID/Permit Form?
You can use any device with Internet capability.
