Can a submission of a minor’s U.S. passport bypass documentation requirements for Responsible Adults?
No. The U.S. passport is an acceptable document to verify the citizenship and identity of the minor. However, it does not show the relationship of the Responsible Adult to the minor. If you are submitting a passport to verify the citizenship and the identity of the minor you must also be prepared to:

Sign the Responsible Adult affidavit (available at the CSC) stating your relationship to the minor; AND

Provide identification to verify your identity; AND

Submit paperwork to show your relationship
