What is a Pauper's Affidavit?

  • 1. Does DDS offer a discount on any reinstatement fee(s) payments?

    DDS offers reduced reinstatement fees by 100% to eligible drivers. The driver must certify that they are unable to pay the entire reinstatement fee. This evaluation is based on the responses provided in the Household Information section of the Pauper’s Affidavit.

    DDS does not offer the 50% reinstatement reduction for the following suspensions:

    • Super Speeder
    • Non-Sufficient Funds
    • Safety Responsibility
  • 2. Who can get a discount on reinstatement fee(s) payments?

    Anyone who owes reinstatement fees can complete the Pauper’s Affidavit. Complete the form, have it notarized, and return it to a DDS Customer Service Center for review.

    DDS does not offer a reduction for:

    • Super Speeder

    • non-sufficient funds

    • Safety Responsibility suspensions.

  • 3. What do I need to do to qualify for a discount on reinstatement fee(s) payments?

    To qualify for a 100% discount on your reinstatement fee(s) you must:

    • Complete and sign the Pauper’s Affidavit. You can get a Pauper’s Affidavit by asking for one at any DDS Customer Service Center. You can also go to the Forms and Manuals page of the website and download the  Download this pdf file. Pauper's Affidavit  form from there.
    • Return the completed and notarized Pauper’s Affidavit to a DDS Customer Service Center by visiting a center in person. You can also mail it to:

  • 4. Why didn’t I get the reinstatement fee(s) payments discount?

    To qualify for the 100% discount you must:

    1. Complete and sign your Pauper’s Affidavit
    2. Have the Pauper’s Affidavit notarized
    3. Return the Pauper’s Affidavit to the DDS either in-person or by mail.

    NOTE:

    • You must meet household size and gross annual income restrictions for the discount. It is a restriction imposed by the Federal Poverty Guidelines created by the US Government.

    • DDS does not offer the 100% reinstatement reduction for:

      • Super Speeder

      • non-sufficient funds

      • Safety Responsibility suspensions.

  • 5.How will my request be evaluated?

    Your request is being evaluated against the Federal Poverty Guidelines as set forth by the Department of Health and Human Services.

  • 6. Can I mail my request for a discount to DDS?

    Yes, DDS will accept your completed, signed, and notarized Pauper’s Affidavit by mail. It MUST include all other reinstatement requirements documents and payment fees together.
