General Information
Effective June 24, 2013, customers needing to obtain a document from a Social Security Administration office may be asked to provide additional documents. Remember that DDS accepts a variety of documents to prove your Social Security Number including W2 forms, 1099 tax statements, tax returns and pay stubs issued by your employer. Since January 1, 2003, no person could be issued a Georgia driver’s license or identification card without providing a social security number at the time of issuance or renewal. Your social security number will remain confidential and will not be disclosed to anyone except as required by law.
Effective July 1, 2012, you must provide documentation with your full number. For a list of acceptable documents proving your Social Security, please view our Secure ID Checklist. The number will be verified with the Social Security Administration (SSA), and the status of your driver’s license may be affected if an incorrect social security number is provided.
For those drivers who are not citizens and not authorized to work in the United States, you will need to obtain a Form SSA-L676 'SSN CARD DENIAL NOTICE' from a local SSA office before visiting a DDS Customer Service Center to apply for a driver's license. You may contact the SSA at (800) 772-1213 or visit their website at https://www.ssa.gov/onlineservices/ and select 'field office locator' to find the SSA office nearest you. Form SSA-L676 is not available from the web site. After obtaining the form, please bring it to the Customer Service Center when you apply for your driver's license or identification card. In addition to the form, please remember to bring your valid passport and valid I-94 permit, and proof of Georgia residency. Verification of Social Security Number
To ensure that each applicant's collected information is correct, the Department of Driver Services (DDS) performs an electronic verification of the first name, last name, date of birth and social security number with information contained in the records of the Social Security Administration (SSA). The information must match exactly, or the status of your driver's license may be affected. If the DDS is unable to verify an applicant's social security number, he or she is notified in writing.
In order to resolve the discrepancy and avoid the cancellation of his or her driver’s license, the applicant must provide corrected information to the DDS within 120 days. Many times an error in personal information can be corrected by having the customer present the proper verifiable information such as an original certified birth certificate. As more complex issues may require more time to resolve, the DDS will grant extensions.

Social Security Administration Announces Additional Safeguards for Obtaining Social Security Number Printouts

Remember that DDS accepts a variety of documents to prove your Social Security Number including W2 forms, 1099 tax statements, tax returns, an SSN Annual Statement and pay stubs issued by your employer. You also may be able to print out your own Social Security Annual Statement by using the Social Security Administration's On Line Annual Statement service at www.ssa.gov/mystatement. *As of August 01, 2014, the Social Security Administration (SSA) will no longer provide printouts.

Individuals will not be allowed to present this document in lieu of the permanent Georgia D/L for identity purposes.
