Get an MVR - Driving History Report (For Individuals)?
There are 3 ways to obtain an Individual Georgia Driving History Report also known as a Motor Vehicle Report (MVR).
1. On the Internet using Online Services
- By creating an Online Account, individuals will be able to request a 3-year or 7-year non-certified or certified MVR.
- You must have a valid credit card for the Internet. We accept Visa, Mastercard, American Express, and Discover.
- Payments can also be made by mail. A payment coupon will be available for print at the end of your transaction. We accept Visa, Mastercard, American Express, Discover, check, cashier check or money order.
2. In-Person at any DDS Customer Service Center
- You can request a 3-year, 7-year, or Lifetime MVR in-person.
- You must fill out the required Motor Vehicle Request Form (Form DDS-18) . The form must be original. Faxes and copiers are not acceptable.
- The full name, date of birth, and Georgia driver's license number of the licensee must be provided.
- The person receiving the report for another individual must show proper identification.
- If you are requesting an MVR for another person, you must get the driver's signature.
DDS Customer Service Centers accepts Cash and Credit/Debit Cards including Visa, MasterCard, Discover and American Express. We do not accept checks, money orders, or cashier checks.
3. By Mail
- You can request a certified copy of your Driving History (MVR) by submitting a letter or
Motor
Vehicle Request Form (Form DDS-18)
requesting either a 3-year, 7-year, or Lifetime MVR. The request should include the following:
- Your full name as it appears on your driver's license
- Driver's license number
- Date of birth
- Complete mailing address
- Self-addressed, stamped envelope

- If you are requesting an MVR for another person, you must get the driver's signature.

Checks, money orders, and cashier’s checks should be made payable to the Department of Driver Services.
Notations and Fees
Notations:
- Non-Certified Driving History Report (MVR) - only available for viewing. It will be available for 30 days after the date requested. A non-certified version can be printed but you will not be able to download it to your computer.
- Certified Driving History Report (MVR) - for employment purposes, insurance agencies, and others that request a version stamped, and certified by DDS. The certified version is not viewable online but will be printed, certified, and mailed to the address that you request. It will reflect your Georgia Driving History on the date it is printed. This report can also be sent to a 3rd party on behalf of the customer.
Fees:
- 3-year Driver History Report $6.00
- 7-year Driver History Report $8.00
- Lifetime Driver History Report $8.00

- The Mobile App allows you to view 2 years of your driving history for free.
- You can also request a 3-year or 7-year, non-certified, or certified MVR report within the app.
