Signature on Application of Minor

What you will need to do when you visit a Customer Service Center (CSC)...

  • Sign a Responsible Adult affidavit (available at the CSC) stating your relationship to the minor; AND

  • Provide a State or Country issued Identification or Letter from State Agency to verify your identity; AND

  • Submit paperwork to show your relationship. Examples include:

    • School documents, tax information, or military documents.

    • Employee ID if you are signing the application as the Responsible Adult and:

      • The minor is in the custody of DHS, DFACS, DJJ and you are an employee of the agency that has custody of the minor

      • You are a social worker working with the minor

      • You are an employee of a homeless shelter or group home at which the minor resides

Common Situations

  • I am a step-parent married to biological parent not listed on the minor’s birth certificate…

    You will need to:

    • Sign a Responsible Adult affidavit (available at the CSC) stating your relationship to the minor; AND

    • Provide a State or Country issued Identification or Letter from State Agency to verify your identity; AND

    • Submit paperwork to show your relationship.  Examples include:

      • Marriage Certificate or license

      • Government-issued documents or correspondence

        • Examples include: school documents, tax information,  military documents

  • Can a submission of a minor’s U.S. passport bypass documentation requirements for Responsible Adults?

    No. The U.S. passport is an acceptable document to verify the citizenship and identity of the minor.  However, it does not show the relationship of the Responsible Adult to the minor.  If you are submitting a passport to verify the citizenship and the identity of the minor you must also be prepared to:

    • Sign the Responsible Adult affidavit (available at the CSC) stating your relationship to the minor; AND

    • Provide identification to verify your identity; AND

    • Submit paperwork to show your relationship

      • Examples include school documents, tax information, military documents.

  • I am the biological parent of the minor but my name is not on the birth certificate…

    • Your minor will still need to submit a document to verify his or her identity. This may be a birth certificate, or U.S. Passport.

    • In addition to that identity document you must also:

      • Sign a Responsible Adult affidavit (available at the CSC) stating your relationship to the minor; AND

      • Provide Identification to verify your identity; AND

      • Submit paperwork to show your relationship

        • Examples include: school documents, tax information, military documents. 

  • Minors who are not required to have a Responsible Adult sign their application for driver’s, permit or identification card…

    The signature of a Responsible Adult is not required if the minor is:

    • An emancipated minor

    • A minor who is validly married

    • A minor on active duty with the armed forces of the United States
